<br /> CONDITIONS OF APPROVAL/REASON FOR DISAPPROVAL:
<br /> 1) Pursuant to LMO Sections 16-3-409, this Tree Approval is subject to expiration. This Tree
<br /> Approval will expire on 22 March 2006 unless a complete development plan review
<br /> application [see Section 16-3-303] is submitted or, where development plan review is not
<br /> required,the approved activity is completed.
<br /> 2) This approval is based on plans drawn by Andrews Engineering dated 10-8-04, revised 2-9-
<br /> 05 to include all conditions of this approval.
<br /> 3) PRIOR TO STAKEOUT INSPECTION, submit revised plans that show the following:
<br /> a. All sidewalks around trees built up from existing grade, including engineering details on
<br /> same.
<br /> b.Eliminate sidewalks in parking islands.
<br /> c.No grading under canopy of 48LO.
<br /> d.No regrading of existing swale under canopy of 48LO.
<br /> e.No landscaping between 48LO and building.
<br /> f. Sturdy wooden barricades around 48L0, 30LO and 50,18L0 as close to driplines as
<br /> possible; barricade all trees to be preserved in parking islands 2 feet behind edge of
<br /> pavement.
<br /> 4) Stakeout inspection prior to clearing. Stake edges of all impervious surfaces, as well as
<br /> pathways of all new utility lines. Tree barricades must be in place for this inspection. Flag
<br /> trees to be removed in red.
<br /> 5) Prior to Certificate of Compliance inspection, have certified arborist root feed, using Mycor
<br /> plus fertilizer, the three live oaks specified in condition 3(f) of this approval. Have certified
<br /> arborist confirm in writing to this office.
